Athletic Pre-Participation Physical Examination Schedule Fall 2008
Requirements: All student-athletes (Grades 7-12) trying out for a Fall sport MUST have a physical examination. NO student-athlete will be allowed to practice with any school athletic team unless first receiving a physical examination. Varsity and Junior Varsity sport practices begin on Monday, August 18, 2008. Modified sport practices begin on Tuesday, September 2, 2008.
Appointment: The physical examination schedule for Fall 2008, is listed below. Appointments need to be scheduled through the Susquehanna Valley Central School Athletic Office at 775-9144. A Susquehanna Valley Central School District Pre-Participation Physical Evaluation form (or a Re-Evaluation form) and a Student-Athlete Participation and Consent form is available in the Athletic Office (or the Main Office of the High School and the Guidance Office of the Middle School).

Students who received a physical examination anytime after the Fall 2007 athletic season do NOT need to schedule a physical examination. A Susquehanna Valley Interval Health History (Re-Evaluation) for Sports Participation Form (White) and Parental Consent form (Gold) is all that is required prior to your first practice and is available for your completion. Return the completed forms prior to August 18, 2008 to the Middle School Nurse, P.O. Box 225, Conklin, New York 13748.
**Parents electing to have their student-athlete's physical examination performed privately do so at own expense. Susquehanna Valley's Parental Consent and Health Appraisal Form must be completed by attending physician. RE-EVALUATION FORMS WILL BE SIGNED ONLY ON THE DATES BELOW!!!
